FWIW, I have occassionally see a 'failure to verify' on my bench setup with a USB converter (out of the many thosands of burns I have done - often hundreds a day while developing the trans code). The code burns but the verify fails. Reburning the code almost always fixes it, and a second reburn will do the trick even in those cases.
In such cases, I believe it is a comms problems in every instance.
