The MS 3.0 harness I have has:
- unpopulated pin 18 - ground. This is how the old harness was documented to be wired, but the new one is documented to have a ground wire.
- heavy gauge wiring for the ignition outputs. This is how the old harness was wired (I assume), but the new one only uses logic-level signals over this wire. So, perhaps this is a hold over.
- I do have two sets of shielded twisted pair for the VR1 and VR2 inputs, and this seems different than the old harness as documented, but perhaps this was modified along the way.
So, it's possible I was shipped the old wiring harness. Anybody with a MS 3.0 have a different wiring setup?

Pin 18 is still unconnected, the wiring diagram image has been updated. This pin is used for sensor ground exactly like pin 20 but is not populated in the harness - the single wire sensor return is all that is needed for the sensors. There are two power ground wires (pins 22 and 23) on the harness, pin 18 is not used for this.

Yes, my apologies, this was entirely my fault in doing the documents and illustrations. Apparently I was having a bad day when I did these, and Bruce and Al have straightened me out (and I promised to pay more attention from now on!).
In addition to pin 18, the GND and Rx connections for the serial connection have also been fixed (though since these are pre-wired to the serial jack, this wasn't noticed as easily by users).

Re: New MicroSquirt wiring confusion
the colors on the revised wiring diagram for the serial still don't jive w/ the pin function list. I really need to know where the Tx, Rx, and Ground should end up on the serial plug. I desoldered the headphone jack and soldered on a USB to UART (TTL) converter to get rid of a connection. I used the original color code on the wiring diagram and suspect I smoked the converter so I'm going to put the headphone jack back on now and would like confermation on what should end up where before smoking another.
Edit: I did some googling and found looking into the serial plug reading from left to right the first top pin is Grnd, third pin is Tx, and 4th pin is Rx. Please give me a thumbs up if thats correct and the MS board 13 is Rx, 14 is Tx, and 19 is ground.

For the serial plug, see the GPIO info (which was the original use of this plug, and still the 'standard' - it follows the Innovate standard). Look here: http://www.msgpio.com/manuals/index.htm (scroll to the bottom).
